General Office Information
Office hours, policies and procedures.
Appointments:
The office of Dr. G. Paul Doxey is located at the St. George Medical Park. Appointments may be scheduled by calling 435-628-3342. Our office hours are 9:00 a.m. to 5:00 p.m. Monday through Friday. Tuesday and Thursday are reserved for surgery, and Dr. Doxey will be in the operating room away from the office. If you need to reach Dr. Doxey, you can leave a message with his staff and he will contact you as soon as possible. Dr. Doxey tries to return calls the same day whenever possible.
First visit:
On your first visit, please bring your insurance, Medicare or Medicaid cards as well as referral patient forms where needed. Plan to arrive about 15 minutes early to fill out our general and medical history forms.
Cancellations:
If you are going to be delayed or will be unable to keep your appointment, please call and let us make the time available to someone else as well as reschedule your appointment.
Delays:
Dr. Doxey realizes that your time is very valuable and he tries his best to stay on schedule, but occasionally emergencies and unexpected delays put him behind. Thorough patient care requires a complete evaluation and it is important to answer all questions a patient may have. At times, this causes delays, but Dr. Doxey hopes you will understand and know that he will give you the same attentive care. Whenever Dr. Doxey is running far behind, his staff will try to notify you. If you feel that you have been waiting too long, please tell the receptionist. Sometimes patients may seem to be taken back out of turn. Usually, they have come to see the audiologist and are not going to see Dr. Doxey.
Telephone calls:
Dr. Doxey’s office staff has been trained to answer many questions, although sometimes you will have a question only he can answer. In these cases, Dr. Doxey’s staff will check with him and return your call as soon as possible. In general, he returns calls after patients are seen – typically around 12:30 p.m. and 5:30 p.m. Urgent calls are given priority and he will return these immediately. If you believe that a medication may be required, please have the phone number of your pharmacy available and make sure they are open.
Prescription refills:
Some medications have refills, which can be obtained from the pharmacist without calling our office. Because many diseases have similar symptoms, please understand that Dr. Doxey may require that he examine you before any medication is prescribed. In general, he will not phone in a medication if he has not seen you for a year or more or if he’s not comfortable with the over-the-phone diagnosis. After office hours, Dr. Doxey does not have access to your records. Please fill him in as to your history, allergies to medications, etc., and remember to have the phone number of an open pharmacy available and ready.
After-hours care and emergencies:
For the most part, Dr. Doxey is personally available 24 hours a day, seven days a week. Occasionally he will have another ear, nose and throat surgeon covering for him. You may reach Dr. Doxey by calling the answering service at 435-656-6065. They will then page him and he will get back to you as soon as possible. If the problem is something that cannot wait, proceed to the nearest emergency room and the doctor there will contact Dr. Doxey. The Emergency Room at Dixie Regional Medical Center has Dr. Doxey’s home phone number and can contact him if his beeper fails. He also generally carries a cell phone.
Billing policies:
Many insurance companies require preauthorization. As a service to you, all insurance is verified and preauthorized by us (even though this is ultimately your responsibility). For this reason, it is essential that you provide us with your insurance phone number and policy numbers. You must verify your own insurance benefits as most insurance companies will not provide us with this information.
Payment is due at the time of service. We accept cash, checks and Visa or MasterCard. In instances where we accept assignment from your insurance, your copayment needs to be paid at the time of service. If you participate with one of the many preferred provider organizations (PPOs) or health maintenance organizations (HMOs) such as IHC care, Valuecare or Healthchoice, it is your responsibility to bring in a referral form from your doctor at the time of your visit. While the filing of insurance claims is a courtesy we extend to our patients, all charges are your responsibility. Regardless of what your insurance pays, your balance, if any, is due in full within 60 days after which an interest charge of 1-1/2% per month will be added. Returned checks are subject to a $15 service charge.
Surgeries:
All surgeries are to be paid in full one week prior to the date of the procedure. As a service to you, we will bill your insurance carrier, after which you will be reimbursed for any overpayment. Cosmetic procedures need to be paid in full a week prior to surgery.
We realize that temporary financial problems may affect timely payment of your account. If such problems do arise, we encourage you to contact us promptly for assistance in the management of your account.
Dr. Doxey is concerned with the cost of medical care and encourages you to ask any questions you may have about his fees, payments and billing policies.
In Conclusion:
My staff and I are glad you have chosen us for your ENT healthcare needs. Although I am a specialist, I am also concerned with the general well-being of my patients. Prevention is an often-overlooked aspect of healthcare. Maintaining a normal weight by following a regimen of eating a healthy diet, exercising regulary, getting adequate rest, and abstaining from smoking are basic health principles that should not be ignored. We strive to provide the best professional care possible. I personally feel that the patient-doctor relationship is one of the most gratifying aspects of the medical profession, and I look forward to caring for you and your family. I welcome any suggestions that will help my staff and me serve you better.
